MEMBERSHIP
Applicant must be a member of an IHA affiliated school.
LIABILITY RELEASE
In consideration of accepting this entry application, I hereby, for myself and my heirs, executors and administrators, forever waive, release and discharge any and all rights and claims for damages or injuries which I may have against the Jung Martial Arts Family Center, Jung Moo Kwan Hapkido, International Hapkido Alliance, its sponsors, employers, instructors, agents, presenters or other participants, for any and all damages or injuries which may be sustained as a result of attending, participating in, association with, practicing for, or traveling to or from, any of it’s activities, training, classes, demonstrations, seminars, tournaments or programs.
It should be noted that martial arts programs are physically aggressive by nature and there is a risk of injury as in all types of training programs. (It is suggested that all participants be examined by a physician prior to beginning any type of exercise program). This authorization wavier includes rendering and accepting any and all medical care, hospitalization or financial obligations incurred as a result of injuries.
VIDEO/PHOTOGRAPH RELEASE
I hereby grant Jung Martial Arts Family Center and Jung Moo Hapkido and it’s presenter’s the irrevocable right and permission to use photographs and/or video recordings of me on websites and in publications, promotional flyers, educational materials, derivative works, or for any other similar purpose without compensation to me.
I understand and agree that such photographs and/or video recordings of me may be placed on the Internet. I also understand and agree that I may be identified by name and/or title in printed, Internet or broadcast information that might accompany the photographs and/or video recordings of me. I waive the right to approve the final product. I agree that all such portraits, pictures, photographs, video and audio recordings, and any reproductions thereof, and all plates, negatives, recording tape and digital files are and shall remain the property of Jung Martial Arts Family Center.
